Might be a good idea to put a note somewhere near the top that one shouldn't use that worksheet with transactions made in a TFSA or RRSP accounts. Might be pretty evident to most, but some might be unsure, or just need a reminder. Also, since potentially a lot of the users might be using Questrade, maybe it would be a nice addition to allow users to upload a file obtained by exporting transactions from the myQuestrade app.
